Process

Permanent License

  1. Submit an application online or mailed in and pay the fee. The processes for RN and LPN licensure are similar. You can begin the RN application here and the LPN application here. The RN application checklist can be found here.
  2. Request verification of licensure via Nursys from your original state of licensure and all other states where you have held a license to practice any profession, active or inactive. If these states do not participate in Nursys, you will need to contact that state's BON and request verification be sent to the New York BON via the Verification of License form.
  3. A “Yes” response to any moral character question on the application requires the applicant to upload an explanation and any court or supporting documents for review. An applicant will be contacted if additional information is required.
  4. Completion of NYS coursework in child abuse identification (CA) and infection control (IC) or a qualified exemption is required, and verification can be uploaded to this application. A list of NYSED-approved providers is available here.
  5. Complete Section I of a Certification of Professional Education (Form 2), and send it to your nursing school, and ask that they complete Section II. Your school must verify your nursing education on Form 2 and then send it directly to the NYSED.

Temporary License

  1. You may submit an Application for Limited Permit at the same time or any time after you submit your Application for Licensure.
  2. Temporary permits are valid for 1 year from date of issue or until ten days after the applicant is notified of failure on the licensing examination.
  3. Temporary permits are generally only granted to RNs who are obtaining licensure by examination, although may be granted to RNs seeking licensure by endorsement under special circumstances for a nurse who is under supervision of a facility.
